Your Health, Our Plan: Understanding Your GP Chronic Condition Management Plan (GPCCMP) 

Living with a chronic condition? We're here to help you manage it effectively. The GPCCMP is a personalised healthcare plan designed just for you. 

What is a GPCCMP? 

It's a structured, written plan created by you and your GP to manage a long-term (chronic or terminal) medical condition, such as diabetes, asthma, heart disease, or cancer. 

From July 1, 2025, this plan replaces the old GPMP and TCA plans. 

Key Benefits for You 

Personalised Care: A plan built around your specific health needs and lifestyle goals. 

Coordinated Support: Ensures everyone involved in your care (GPs, nurses, allied health) is on the same page. 

Access to Allied Health: Receive up to 5 Medicare-subsidised sessions per calendar year with professionals like physiotherapists, dietitians, or podiatrists. 

Clear Goals & Actions: Understand your health goals and the steps you need to take to achieve them. 

What the Plan Includes 

Your GPCCMP will detail: 

• Your health conditions and needs 

Goals you've agreed upon with your GP 

Actions for you to take (e.g., exercise, diet changes) 

Treatments & Services you are likely to need 

Referrals to other health professionals 

• A date for the next plan review 

The Process: What to Expect 

1. Discussion: Talk to your GP about your chronic condition and your health goals. 

2. Agreement & Consent: Agree on the plan's details and give your consent to share it with your care team. 

3. Receive a Copy: You will be offered a copy of your completed plan to take home. 

4. Regular Reviews: We will schedule regular reviews (usually every 3-6 months) to track progress and update your plan.
